This is a temporary position Not To Exceed (NTE) September 30, 2021. The registered nurse assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Duties include, but are not limited to: Provides a full range of nursing care to patients with complex care needs utilizing the nursing process based on the ANA Standards of Nursing Practice. Accurately documents in the medical record patient findings, assessments, and care provided. Effectively functions as the charge nurse, team leader, or in other leadership role that guides and directs others who provide patient care. Uses a team approach to identify, analyze, and resolve patient care problems that result in improved patient outcomes. Role-specific Competency Checklist is complete and is rated competent in all measured areas. Practice follows the ANA Code of Ethics for Nurses. Identifies potential and actual problems involving resources and/or safety. Effectively plans patient care and nursing resources for maximum staff productivity, staff and time efficiency, and positive patient outcomes. Consults with other disciplines for post-discharge care. Demonstrates leadership with compliance with infection control practices, such as hand washing and wearing of appropriate personal protective equipment. Perform other duties as assigned. Work Schedule: Monday - Friday 8:00 a.m. - 4:30 p.m. Financial Disclosure Report: Not required
