Clinical Pharmacist
DescriptionSummary:The Clinical Pharmacist shall provide the necessary pharmaceutical services needed to facilitate the physician in the treatment of the patient. Such services shall include medication order review, monitoring drug regimens including food/drug and adverse interactions, consulting with providers, nursing staff and patient/family concerning drug therapy, preparation of sterile admixtures, preparation of cytotoxic agents, and providing assistance during “Code Blue� codes. The Clinical Pharmacist will participate in daily interdisciplinary rounding on their assigned unit or where necessary to facilitate pharmacy/nursing/physician collaboration.
