As the Quality and Performance Improvement Clinical Pharmacist Specialist, you will be responsible for the development, implementation and coordination of Pharmacy related quality, accreditation, and Performance Improvement programs (PI) for Pharmacy Services.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Working with Pharmacy Leadership, develop, implement, and maintain policies and procedures to ensure optimal quality, safety, compliance and operational performance
Ensures that accepted standards of practice as outlined by accrediting bodies, ASHP, USP, and ISMP are followed.
Participate in all Pharmacy related surveys to provide expertise in quality and performance improvement aspects
Keeps current on industry standards and regulatory/compliance issues within Pharmacy
Provide proactive reporting consultation to Pharmacy Leadership team on regulatory and compliance issues
Leads efforts in continuous process improvement to maximize efficiency, safety and ongoing survey readiness for the department and health system
Defines the processes of data collection and analysis that lead to quantification of departmental impact on patients and health system resources
Provide oversight for staff training and competency assessment for the department
Lead departmental performance improvement projects and routinely monitor and report findings to the associated Pharmacy Directors
Develops strategies for efficient performance while minimizing the potential for error
Lead departmental ESP (Effective & Safe Pharmacy) committee
Participate in research design, data analysis, and publication of study results for research projects and medication use evaluations
Assist or directly precept residents and pharmacy students
Maintain a mastery of knowledge related to current resources and standards pertinent to pharmacy quality, accreditation and performance improvement
EDUCATION AND EXPERIENCE REQUIREMENTS:
ASHP-accredited PGY1 Pharmacy Practice Residency required
ASHP-Accredited PGY2 specialty residency or 5 years of relevant experience required (At least 5 years as a Pharmacist and at least 3 years in performance improvement)
Board Certification in Pharmacotherapy is preferred
Six Sigma/LEAN Six Sigma certification or Vizient Analyst Certificate program required or must obtain within 2 years of hire
Delaware Pharmacist Licensure or eligible for reciprocity
