The Infusion Clinic Registered Nurse (RN) functions directly to support the activities of Primary and Specialty Medicine. The RN is responsible for the planning, integrating, implementing, modifying, and evaluating the effectiveness of the Veteran's care. The RN is the lead of the integrated care team established by the Primary and Specialty Medicine. Major duties include, but are not limited to: Provides leadership in the application of the nursing process and identifies resources and critical factors for achieving desired outcomes for discharge, health maintenance/improvement and the identification of resources and support during periods of declined health. Provides initial and ongoing assessment of patients to identify needs and appropriate resources and/or interventions. Recognizes complex situations that impact patient care and offers interventions, using sound judgement, professional attitude, and appropriate channels. Recognizes the impact of age-specific needs and incorporates this into the assessment process. Incorporates these age-specific needs into care as reflected by modification of treatment plans as appropriate. Supports and enhances patient self-management and determination. Support mission and vision to include but not limited to Health Promotion Disease Prevention, Tele-health, and patient center care. Utilizes appropriate resources and tools to document, track, and assure follow through on patients with high risk needs or care management coordination. Serves as a member of the Code Blue response team. Coordinates/performs outpatient treatments, interventions, and implements appropriate protocols. Assumes responsibility for the seamless transfer of the patient from the St. Cloud VA Health Care System to receiving facilities. Work Schedule: Monday-Friday tour of duty between the hours of 8:00am-4:30pm; Expected to work up to 30 hours/week with flexible shifts.
