Job Summary We are seeking a dedicated I-CORT Registered Nurse to join our dynamic healthcare team. In this vital role, you will provide nursing care within the Intensive Crisis Outreach and Response Team (I-CORT), delivering compassionate support to individuals experiencing mental health crises. This position offers an exciting opportunity to make a meaningful difference in people's lives through proactive, patient-centered care.
Duties
Provide services to persons with severe and persistent mental illness who qualify under the Intensive Community Outreach and Recovery Team (I-CORT) program and serves as a member of the I-CORT multidisciplinary team.
Provide support to individuals, both on a planned and as-needed basis, to help them accomplish their personal goals, to cope with the stressors of day-to-day living, and to reach goals as identified on the ISP and Recovery Support Plan.
Provide nursing assessments in compliance with DMH standards and within the scope of Mississippi Nursing Practice Acts and Rules and Regulations.
Evaluate client’s response to medications i.e. effectiveness and/or side effects and documents appropriately.
Administer medication injections as prescribed by physician/PMHNP and follows all other orders.
Provide general health care monitoring, supportive counseling, recovery/resiliency support, and symptom management.
Provide services in the individual’s natural/community setting 75%-85% of the time.
Participate in daily organizational meetings to review service contacts within the past 24 hours and action plan for the current day.
Participate in agency MAP teams and staffings when indicated.
Maintain case record, appropriate activities, and service log documentation that is adequate, appropriate, complete, and timely.
Develop the Recovery Support Plan with input from the multi-disciplinary team. Assists in development of the Individual Service Plan and Crisis Support Plan.
Rotating on call schedule to respond to needs of individuals enrolled in the I-CORT program.

Required
Licensed to practice as a Registered Nurse in the state of Mississippi.
Valid driver’s license and auto liability insurance.
Excellent communication skills with the ability to build rapport quickly and work collaboratively within multidisciplinary teams
Preferred
Previous experience in a mental health setting strongly preferred.
Previous experience working with an SMI population in a treatment setting strongly preferred.
